The “HEALTH AT HOME” initiative aims to provide a qualified response in formal support in the home context, in caring for the elderly (+65 years), offering the possibility, for those who wish, to grow old at home, with comfort, safety and autonomy in an integrated manner.
We provide Integrated Support at home, combining diversified care, according to the specificity of each user, supported by new technologies, alongside combating isolation and delaying institutionalization. The focus of the intervention is disease prevention (in its physical and mental aspects) and health promotion.
It is aimed at seniors who want to monitor their health status, with/without diagnosed chronic diseases, polymedicates, with or without SAD or other support.
